Your First Visit
Please arrive about 15 minutes early to complete any paperwork. Bring a photo ID, your insurance card, a list of current medications, and any relevant imaging or records from prior providers.
What to Bring
- Photo ID and insurance card
- List of current medications and allergies
- Comfortable shoes you wear most often (helpful for gait assessment)
- Any prior X-rays, MRIs, or medical records
Insurance & Billing
We accept most major insurance plans. Because coverage varies, we recommend confirming your specific plan with our office before your visit by calling 940-260-7741.
In-office procedures are performed onsite, while larger surgical procedures are coordinated with nearby accredited surgery centers and hospitals.
